Exemption card application or renewal for disability service workers (DWS/E)

This form is to be completed by Queensland registered teachers and police officers proposing to start or continue in child-related employment.

Paper applications which are valid for lodgement until 30 June 2024 will continue to be accepted until further notice.

Completing a paper form requires you either:

verify your identity online and include your account number on the paper form
provide a certified copy of your ID document. For example, a driver's licence, photo identification card or marine licence indicator.
